
When the cornea becomes irregular, scarred, severely dry, or otherwise compromised, standard contact lenses may not provide stable vision or lasting comfort. Scleral lenses use a different design. Instead of resting directly on the cornea, they arch over it and land on the sclera, the white part of the eye.
This structure creates space for a layer of preservative-free saline between the lens and the corneal surface. The result is a smooth optical surface and a protective liquid reservoir that can support clearer, more comfortable vision.
A scleral lens is larger than a conventional soft or rigid gas-permeable lens. Its rigid, dome-shaped design is customized to fit the contours of the eye while maintaining clearance above the cornea.
Before insertion, the lens bowl is filled completely with preservative-free saline. Once the lens is placed on the eye, the saline remains trapped beneath it. This fluid-filled space continuously bathes the cornea during wear while the lens rests on the less sensitive scleral tissue.
Because the lens does not move across or press directly against the damaged corneal surface, it can reduce mechanical irritation. Its stable position also creates a consistent front surface that helps light focus more evenly.
The saline reservoir serves several important purposes:
• Keeps the corneal surface continuously hydrated during lens wear
• Shields sensitive tissue from air, dust, debris, and eyelid friction
• Masks corneal irregularities to improve visual clarity
• Creates a smooth optical surface for more stable vision
• Supports comfort when standard contact lenses are difficult to tolerate
This liquid shield does not repair every corneal condition on its own, but it can create a protected environment that reduces exposure and dryness. Patients must still receive appropriate care for the underlying cause of their corneal damage.
Scleral lenses are often considered for people with keratoconus, corneal scarring, severe dry eye, irregular astigmatism, complications following eye surgery, or other ocular surface conditions. They may also help patients who cannot achieve clear or comfortable vision with traditional contacts.
Every eye has a different shape, especially when the cornea is irregular. A precise scleral lens evaluation is needed to measure the eye, determine the correct vault, assess the landing zone, and confirm that the lens provides adequate clearance without excessive pressure.
A successful scleral lens fit balances vision, comfort, hydration, and corneal health. The lens must hold enough fluid to protect the cornea while fitting securely over the eye.
Follow-up visits allow our eye doctor to evaluate lens settling, check for pressure points, monitor corneal health, and make adjustments when necessary. Patients also receive instructions for insertion, removal, cleaning, disinfection, and the safe use of preservative-free filling solution.
